Coordinator Rapid Rehousing

As Rapid Rehousing Coordinator I oversee The Center’s Rapid Rehousing Program (RRH) for Transition Age Youth
(TAY). This program assists homeless TAY (ages 18 – 24) in obtaining and maintaining permanent housing. I supervise
the RRH Specialists and the RRH Landlord and Employment Liaison to assist clients in apartment searches, working with
landlords, moving into and setting up their apartments, and other activities to ensure self-sufficiency and housing stability.
Also, as RRH Coordinator I’m responsible for maintaining the fiscal and operational components of this HUD-funded
program. Oversight of the development, implementation, ongoing assessment and coordination of client case
management needs, services and plans; data collection, outcome measurement, record keeping and report generation;
Furthermore, I also participate in the On-Call RHY Supervisor rotation (for 1 week, every 5 - 6 weeks). This emergency
response system is intended to provide after-hours and weekend support to the Emergency Shelter and Transitional
Living Program. Moreover, I managed time sheets, conducted weekly staff meetings as well as weekly supervision
meetings with my staff, implicated and development program forms, and

Program Research Specialist 3 (PRS3)

As PRS3 I provide the DOCCS Research and Planning Unit with Title I data, re-entry preparation data, academic achievement data, workforce data and attendance data. I carry out the evaluation and reporting requirements, In keeping with the program evaluation plan, assessment data is collected and utilized weekly, monthly, quarterly and annually. I also prepare written technical reports containing the analysis and interpretation of study findings along with recommended course of action based on study findings. Furthermore, I participate with the Title 1 Grant planning and writing. As a NYS DOCCS employee, I have attended trainings in order to develop skills in creating an environment that promotes respectful professional and interpersonal abilities. Transitional Living Program Case Manager
As case manager I meet with transitional living residents (youth) on a weekly basis and non-residents twice a month. Clients are between the ages of 16-21. Overall the goal is to help the youth become self-sufficient. As Case manager I’m responsible for assisting clients in navigating the systems. Help clients communicate with professionals to overcome social and cultural barriers. Provide clients with skills such as budgeting, time management, developing short- and long-term goals and communication as well as providing clients with tools (life skills) and resources, so they can feel confident, knowledgeable and capable of success. Through regular meetings, as case manager I encourage and motivate my clients to work on their goals and focus on the individual steps required to achieve goals. Goals might include graduating from high school, passing the GED and preparing for and gaining employment. Furthermore, as case manager I transport clients to appointments on a need be basis. Provide crisis intervention when needed. Provide after care services to youth and their families when the client leaves the program. Perform assessments of the youth to determine services needed. Perform intakes into the program.

Conduct community research with Project HOPE’s new initiative to deter open-air marijuana markets in the northeast side of the city. Also took part in group meetings, conducting surveys, compiling data charts, resident outreach and youth outreach, and community presentations for Project Hope staff members. Lead outreach team, conducted outreach meetings with other organizations, created outreach flyers, and created outreach summer plan as coordinator.
