Cardiac Medical Unit
Assess patient and document findings. Report any change in patient condition or lab values to the physician. Obtain and follow orders as given. Monitored all cardiac patients which include; chest pain, non-stemi MI, post MI, CHF, and cardiomyopathy. Initiated, monitored, and safely titrated infusion of appropriate medications. Monitored and assessed pre and post cardiac intervention patients per protocol. Assist physician in removal of cardiac sheath. I provided written and verbal education to the patient from the time of their admission to the floor, until their time of discharge. Prepared assignment for the next shift based on the acuity of the patients.

Hospice of the Western Reserve: 06/2008 – 06/2009
Plan of care was initiated at the time the patient was admitted to the program. We worked together (physician, social worker, spiritual care advisor, home health aide) as a team, to ensure that the best emotional support and physical care was provided to not only the patient, but also the family. I ordered and arranged for the delivery of medications (with prescription from physician) and medical equipment that was needed in the home.
